You can request a copy of the accident report at the precinct office during the 30 days following your accident. The process is straightforward. Simply complete a REQUEST FOR COPY OF ACCIDENT RECORD [PD301-165] and pay a fee of $10, either check or money order, payable to “City of New York, Police Department.”
After 30 days, you can request a copy of the accident report from the New York State Department of Motor Vehicles. Complete the NYS DMV form MV-198C and pay a fee of $20.
